The six (6) sets of actions envisaged or the successful completion of the study are the following:

Deliverable description
Report on the results of the pilot implementation of the multichannel online platforms as well as on its full implementation and user manuals. Digital material from the platform will also get produced
Technical report (and digital material) on the finally formed multi-channel platform’s potential and user instructions

Deliverable description
Technical report documenting the choice of clinical parameters to assess the severity of patients' health status during hospital admission, their health outcomes and disability level
The completed Case Report Form including an and the identification manual of the various variables
A publication of the descriptive analysis of the study population
