1. Onset of CVA (Cerebrovascular accident): 1.000 patients with first-time stroke.

2. Arrival at Emergency Department. Logging of the following parameters:
⦁ Basic demographic & clinical characteristics of the patient
• Specification of CVA type
• Medical history
• Neurological status evaluation according to the National Institutes of Health Stroke Scale (NIHSS)
• Imaging procedures & type of intervention during hospital admission

3. Inpatient Hospitalization. Logging of the following parameters:
• Data related to acute phase treatment (e.g. thrombolysis or thrombectomy)
• Data of diagnostic control, treatment and monitoring of the patient during the overall duration of the hospitalization (imaging findings, laboratory testing, pathophysiological classification according to TOAST classification, medication)
• Data of resource consumption (medicines, materials & consumables, laboratory tests) and use of early rehabilitation services during hospitalization
• Staff experiences and Professional Quality of Life (ProQOL questionnaire)

4. Hospital Discharge. Logging of the following parameters:
• Length of hospital stay and destination following hospital discharge
• Medication following hospital discharge
• Level of disability and functional rehabilitation of patients (NIHSS, mRS) – patient status
• Patient experiences of inpatient hospitalization

1 month following hospital discharge: Logging of the following parameters (the revaluation of the patient takes place one month following hospital discharge by telephone):
• Clinical outcomes (e.g. mortality, stroke recurrence, cardiovascular and / or hemorrhagic events, assessment of function/disability level – mRS scale)
• Area of residence and rehabilitation
Telephone communication by an independent team of researchers (non members of the medical team) to record patients’ experiences of inpatient hospitalization.

3 months following hospital discharge: Logging of the following parameters (the reassessment of the patient takes place three months following hospital discharge during their visit at the outpatient clinic for re-evaluation):
• Clinical outcomes: (e.g. mortality, stroke recurrence, cardiovascular and / or hemorrhagic events, assessment of function/disability level – mRS scale)
• Patient’s quality of life (EuroQoL questionnaire)
• Assessment of adherence with taking and refilling medication (Adherence to Refills and Medications Scale-ARMS)
• Adherence with the proposed treatment regimen and assessment of certain hygienic diet parameters
• Area of residence and rehabilitation
• Caregiver’s quality of life (EuroQoL questionnaire)

12 following hospital discharge: Logging of the following parameters in a sample of 300 participants (Reassessment of the patient takes place 12 months following hospital discharge either by visiting the outpatient clinic or by telephone):
• Clinical outcomes: (e.g. mortality, stroke recurrence, cardiovascular and / or hemorrhagic events, assessment of function/disability level – mRS scale)
• Medication adherence
• Patient’s quality of life (EuroQoL questionnaire)
• Area of residence and rehabilitation
• Caregiver’s quality of life (EuroQoL questionnaire)
• Impact of the illness on patients and their family members relationships
• Completion of a questionnaire on socioeconomic consequences of the illness (data on loss of productivity, caregivers’ needs, estimation of the family budget burden due to the illness, etc.)
